Terms and conditions for Maternity Leave Plan

Unlimited classes for six consecutive weeks, starting on the day the plan is first used. This plan also includes a five-pack of classes to use at the completion of the six weeks. Five-pack of classes are to be used within 6 months of start date. If an extension is needed you must reach out to Kellie before they expire. kelliefell@fit4mom.com
