Terms and conditions for Unlimited Monthly Membership: Month-to-Month
- Regularly priced monthly membership is $89 per month. If purchasing at a discounted rate, the first month will be discounted and the following month will be charged at the regular rate. If you wish to cancel, you must email the owner, Kellie Fell, at kelliefell@fit4mom.com.

- Monthly fee will be charged to your credit card automatically every month
- Autopay will continue until you cancel your membership
- To cancel or put your membership on hold you must notify the owner at least 2 weeks before membership is set to charge for the following month. If this requirement is not met, the client is responsible to pay the upcoming month's membership dues.
- Please note we may close between Christmas and New Year's Day, as well as other holiday times.

This contract is made between the client and FIT4MOM Westside and is a month-to-month agreement and except as provided in the initial 10-day cancellation policy below, may be terminated by either party with the proper advance notice. This membership allows you to attend Stroller Strides, Stroller Barre, Strides 360, and Body Ignite classes unless otherwise specified.
AUTHORIZATION FOR PAYMENT
I authorize FIT4MOM Westside to initiate a charge to the account entered into Front Desk/Pike 13 for my dues. This authorization is to remain effective until I have canceled my membership through the FIT4MOM Westside administrator.
I have the right to stop payment on an Electronic Funds Transfer debit by notifying my bank. This, however, does not void my contract with FIT4MOM Westside to fulfill my payment commitment and I am obligated to pay. Dues will be drawn monthly from the account on a monthly basis which will occur the day the membership began. The processing date for debit/credit cards may vary due to banking procedures. If charges are returned they will be subject to a late fee. Members are responsible for updating their credit card information online. Members may not attend class who have unpaid balances. If a membership remains unpaid it will be canceled.
CANCELLATION POLICY
To cancel this membership, the member must contact the FIT4MOM Westside administrator with at least 2 weeks notice, prior to their next payment due date to request a cancellation. No refunds will be given for dues once it has been paid. This membership is not transferable to another person or anywhere outside the Westside franchise. Once a membership is canceled, if the member wants to re-enroll they may be charged a $25 fee.
HOLD POLICY
Memberships may be held a maximum of 4 months in any given 12 month period. Hold start dates can be requested in writing only. When a hold period is ended, the membership will reactivate or may be canceled at the owner's discretion.
INITIAL 10-DAY CANCELLATION POLICY
You, the buyer, may cancel this agreement at any time prior to midnight of the tenth (10th) calendar day after you have signed this agreement. To cancel this agreement written notice must be received by FIT4MOM Westside within the initial ten days. You are responsible for paying for the classes already attended.
AGREEMENT
By purchasing this contract, you are agreeing to the terms of this contract. Terms of this agreement may change with the proper advance written notice.
FIT4MOM OnDemand Monthly Subscription
With your paid monthly membership you are given free access to FIT4MOM OnDemand. You will have unlimited access to OnDemand workouts. When your FIT4MOM membership is properly canceled according to the Terms and Conditions, your OnDemand subscription will be automatically canceled. If you would like to keep your OnDemand subscription you can re-enroll using the same login information and purchase your desired paid subscription.
